An AI Incident is any event where a deployed AI system causes harm, fails in production, or generates an output materially wrong enough to require remediation or disclosure — ranging from a hallucinated customer response to a biased automated decision to a full model outage. Incident volume and severity, weighted by source tier, is the direct input to the Index's inverse-scored Incidents dimension.
